- Live in any state other than: Hawaii, Alaska, Wisconsin, New York, Connecticut, Massachusetts, Rhode Island, New Hampshire, Minnesota, Vermont
- age 64 years 6 months to 65 years 5 months any health conditions anytime
- age 65 years 6 months or older any health conditions if applying between October 15 and Dec 7
- age 65 years 6 months or older no serious pre-existing conditions if applying outside Oct 15 and Dec 7 and not currently on Medicare Advantage for more than 12 months
- anyone anytime 3 months or less before Medicare Part A and B effective date any health condition
- age 21 to 64.5 years excellent health anytime
